Peak Brain Texas
Privacy Policy
This policy explains what the Texas website and inquiry process collect, how the information moves through Peak Brain systems, and the choices available to you.
Effective date: August 20, 2026
Policy version: 2026-08-20
The required inquiry checkbox permits us to store and use the information you submit to respond, send requested program and pricing information (including related follow-up offers), and coordinate follow-up. The general announcements checkbox is separate and optional.
1. Who We Are
Peak Brain Texas, operating through peakbraintexas.com, is a regional affiliate of Peak Brain Institute. The Texas team and authorized Peak Brain Institute personnel use shared systems to respond to inquiries and coordinate services.
- Location: 4965 Preston Park Blvd, Suite 700, Plano, TX 75093
- Regional lead: Vijay Dukkipati
- Privacy contact: info@peakbraintexas.com
2. Information the Inquiry Form Collects
The Peak Brain Texas consultation form may collect:
- First and last name, email address, phone number, and city or region
- Program interest and preferred contact method
- How you heard about Peak Brain and any referral details you provide
- An optional free-text message
- Optional goal selections, such as attention, stress, sleep, mood, concussion, trauma, or migraine
- Whether you separately opted in to announcements
- The privacy-policy version and consent language presented when you submitted
- Submission timing and operational records needed to process the inquiry
This general inquiry form is not the full client intake or an emergency channel.
Do not submit medical records, payment-card details, or other information you do not want included in the shared client system. If you may be experiencing an emergency, contact local emergency services.
After a program agreement, the Texas onboarding page embeds the existing shared Peak Brain intake. That questionnaire is delivered through Zoho Creator and may collect health, history, goals, and other program-preparation information. The Zoho transfer is disclosed directly above the embedded questionnaire.
3. How We Use Inquiry Information
- Respond to your question using the contact information and preference you provide
- Send the pricing or program information you requested, including related follow-up offers, and coordinate appropriate follow-up
- Schedule consultations and support local or remote program onboarding
- Create or update a contact, submission, activity, and interest record in the shared Peak Brain CRM
- Place the inquiry in the applicable Peak Brain Texas lead list or requested follow-up sequence
- Send general announcements and event updates only when you select the optional announcements checkbox
- Operate, protect, troubleshoot, and improve the website and inquiry process
Peak Brain Texas does not sell or rent the personal information submitted through this form.
4. Systems and Service Providers
Information is available to the Texas team and authorized Peak Brain Institute personnel who need it to respond or provide services. The website also uses the following providers for specific functions:
- Vercel: website hosting and delivery.
- Supabase: the shared database and CRM records for inquiries, contacts, interests, consent, activities, and purchases.
- Zoho Creator: the embedded shared Peak Brain client intake questionnaire and client records used to prepare for and provide services.
- ReachInbox: Peak Brain Texas lead lists, requested program and pricing follow-up (including related offers), free-practice updates, and optional general announcements.
- Slack: a staff-only operational notice containing name, contact details, program interest, location, contact preference, lead source, and referral details. Optional goals and the free-text message are not placed in that Slack notice.
- Stripe: embedded payment processing, receipts, and purchase status when checkout is enabled.
- Calendly: scheduling when a Calendly booking interface is displayed or used.
- SiteBehaviour: optional website-use analytics loaded only after cookie acceptance.
These providers process information for the functions described above and may have their own privacy terms. We may also disclose information when legally required or when necessary to protect the website, our users, or Peak Brain operations.
5. Email and Follow-Up Choices
The required inquiry consent is used for direct responses, requested program and pricing information, related follow-up offers, and coordination connected to the program interest you selected. It is separate from the optional general-announcements consent.
If you opt in to general announcements and event updates, you may unsubscribe using the link in an email or by contacting us. You can also ask us to stop non-transactional follow-up at any time.
6. Purchases, Agreements, and Payment Information
When embedded checkout is enabled, Stripe collects payment-card and billing information. Peak Brain Texas receives transaction confirmation, customer contact information, the selected program and delivery options, payment status, and identifiers needed to record and fulfill the purchase. This website does not store the full payment-card number.
A signed program agreement is stored in the shared Peak Brain client system with the agreement version, selected services, acknowledgements, signature record, date and time, and technical evidence used to protect the integrity of the electronic record. You may request a copy from info@peakbraintexas.com.
7. Cookies and Website Analytics
The cookie banner lets you accept or decline optional analytics. SiteBehaviour loads only after the browser records an acceptance. Declining optional cookies does not prevent you from viewing the site or submitting an inquiry.
Hosting and security systems may still process technical request information needed to deliver and protect the website. You can also manage cookies through your browser settings.
8. Retention, Security, and Your Choices
We retain inquiry and transaction records for as long as reasonably needed to respond, provide services, maintain operational records, resolve disputes, and meet applicable obligations. We use administrative and technical safeguards appropriate to the systems involved, but no internet transmission or storage system is guaranteed to be completely secure.
You may ask what contact information we hold about you or request a correction, deletion, or communication preference change by emailing info@peakbraintexas.com. Some transaction or operational records may need to be retained where required.
9. External Links and Policy Changes
This site links to Peak Brain Institute and other external services. Their privacy practices apply after you leave peakbraintexas.com. We may revise this policy as the Texas operation and its systems change; the effective date and version at the top identify the policy currently presented with the inquiry form.
